The 2016 open enrollment period for choosing health insurance coverage through the federal Marketplace has ended. But if you have a special circumstance, you may still be able to apply.

If you or someone in your household lost insurance, got married, had a baby, had a change of income, or some other special circumstance, you may still qualify. Fill out this brief survey to find out.
