Printed form. The main text in Fraktur is in the center of the document. In the upper center is a circular framed image of two parents with a child in a cradle. Above the image is a bird. In the upper corners are two angels holding wreaths. Leafy vines of flowers are around them. The text is flanked by two images in arched frames. On the left is a couple getting married by a minister. On the right is a couple with a minister who is baptizing their child. Both pictures are topped by garlands. In the lower center is a circular frame with a woman grieving over a tombstone in a graveyard. Above the image are two praying angels. In the lower corners are leafy vines and flowers.
