Translation:
Jacob Roth was born the 7th of December 1780 in Maxatawny Township, Berks County, in the state of Pennsylvania. The parents are Michael Roth and his wife Elisabetha, a born Beckerin. He was baptized by the honorable Mister Philipp Jacob Michel, evangelical Reformed preacher. The sponsors were Jacob Becker and his wife Maria. In the year 17 [blank] the same was instructed in the Lord’s Supper and confirmed by Mister pastor [blank].

Hand-drawn; hand-colored; hand-lettered. The main text in Fraktur within a rectangular frame is in the center of the document. The two verses in German script within rectangular frames are below it. The text is surrounded by various compass-drawn stars and geometric motifs that are enclosed by square and rectangular frames.
The two verses are by Johann Jacob Rambach (1734).
