Translation:

Birth and Baptismal Certificate
To these two married people, namely Peter Riem and his lawful wife Rebecca Riem, born Gerret, a son was born into the world, on the 31st day of July in the year if our Lord Christ 1829. He was born in Schäfferstown, in Lebanon County, in the state of pennsylvania in North America, and received in the holy baptism the name Israel on the 22nd day of August in the year 1829, from the honorable Mr. Pastor Leimbach. The sponsors were Johannes Gerret and his lawful wife Sara Gerret.

[in heart in upper left]
Shepherd, accept your sheep. Head, make it to one of your limbs. Heaven’s path, show it the way. Lord of peace, grant it peace. Grapevine, help so that this vine will also surround you in its faith.

[in heart in upper right]
You yourself will dress it in truth and decide before the godless herd, to grant the true faith that can guide heart and head to the heavenly, from this world.

[in frame in lower left]
I am baptized, I stand united with my God through my baptism. I therefore always speak joyfully in hardship, sadness, fear and need. I am baptized, that’s a joy for me. The joy lasts eternally.

[in frame in lower right]
I am baptized, and when I die, how can the cool grave hurt me? I know my fatherland and legacy that I will have with God in Heaven. After death, Heaven’s garment of joy and celebration is prepared for me.

[in lower center]
I am baptized in your name, God, Father, Son and Holy Ghost. I am counted as one of your seeds, to a people that you blessed. O! What fortune for me! Lord, let me be worthy of it!
