Hand-lettered, printed form decorated with lithographed decoration and letterpress printed type.  The main text in Fraktur within a rectangular frame is in the center of the document. In the four corners are oval frames in a floral design with references to Bible verses. Four biblical images are above, below and next to the text. The image on top depicts Jesus preaching to the people and healing the sick. On the right side is Jesus rising up to heaven. In the lower center is an image of Jesus’ baptism in the River Jordan, with a dove flying above John the Baptist, and several people looking on. The image on the left shows Jesus on the cross.

Heinrich Keyser, a schoolmaster in Lancaster County, filled out the certificate and signed his name and the date 1845 at the lower right.  The printer of this form is anonymous, but it was closely copied from an Alsatian form printed c. 1838 and lithographed by Godefroy Engelmann.  For examples of this form, see FLP 559 and 1204-1206.
