Translation:

I, Isaac Lentz, was conceived and born to Christian and honest parents on the 24th of September 1792 in the sign of Capricorn, in Pennsylvania, in Northampton County, in Williams Township. The father was the honorable Peter Lantz and the mother Catharina, a born Reimerin, and was baptized in the Reformed religion by Mister Jäger. Sponsors were Isaac Schumacher and his wife Catharina.

Hand-drawn; hand-colored; hand-lettered. The text in Fraktur within a heart is in the center of the document. In the lower corners are two birds and flowers, probably printed by a woodblock stamp. In the upper corners are two tulips. In the top center and below the heart are rosettes.
