To these two married people namely [blank] and his lawful wife [blank] a born [blank] a [blank] was born into the world on the [blank] 18 [blank] in the year of our Lord [blank] This [blank] was born in [blank] County, in the state of [blank] in North America, and was baptized and received the name [blank] on the [blank] in the year of our Lord 18 [blank]. The sponsors were [blank].

Go ye therefore, and teach all nations, baptizing them in the name of the Father, and of the Son, and of the Holy Ghost. Teaching them to observe all things whatsoever I have commanded you: and, lo, I am with you always, even unto the end of the world. Matthew 28: 19,20.

God, this child may be recommended to you. You accept it, your word is true. We are offering it to you in the holy baptism, as you have commanded. O, bless it, father, son and ghost, as your dear word promises.

Hand-colored; printed form. The main text in Fraktur within a decorative frame is in the center of the document. The frame is topped by two angels and two urns with flowers. Above the main text is an image of Jesus in the manger. He is surrounded by several individuals who are praying to him. On the left side of the document is an image of John the Baptist. On the right side is a women dressed in long robes. In the lower center is Jesus receiving his baptism from John the Baptist. Six verses in Fraktur are in the four corners and in the lower center.
