Geist Gottes, selbst mein Beistand sein.

[on right]

Birth and Baptismal Certificate
To these two married people, namely [blank] and his lawful wife [blank], a born [blank] a [blank] was born into the world, the [blank] in the year of our Lord 18 [blank]. This [blank] was born in [blank] County, in the state of [blank] in North America, and was baptized and received the name [blank] in the year of our Lord 18 [blank] from Mister [blank]. The sponsors were [blank].

[on the left]
Therefore go forth, and teach all nations, and baptize them in the name of the Father, and of the Son, and of the Holy Ghost: And teaching them to observe all things whatsoever I have commanded you: and, see, I am with you always, unto the end of the world. Matthew 18. 19. 20.

I am baptized into Your Name, God, Father, Son, and Holy Ghost! I am counted as one of Your seeds, to a people that You cherish. I am buried with Christ; I have been bestowed with Your spirit.

My loving Father, You have declared me to be Your child; my faithful Savior, You have offered me the fruits of Your death; You want me to dedicate my life to God, God’s Spirit will be my comfort.

[on the right]
And Jesus, when he was baptized, went up straightway out of the water: and, see, the heavens were opened unto him, and he saw the Spirit of God descending like a dove, and lighting upon him. Matthew 3: 16.

God, to You this child may be recommended You accept it; Your Word is true. We are offering it to you in the holy baptism as you yourself prescribed. Oh, bless it, Father, Son, and Ghost, as your dear word promises us.

Through Your mercy’s seal, every right will be that of Christians. Consecrate us in the water bath to be your child and heir. O holy, may it once know the value of the joy that it belongs to you.
