Translation:

This little harmonious melody book belongs to Anna Lädtermännin, singing student in the Deep Run School. Written the 12th of January anno 1812.

Notes:

Hand-drawn; hand-colored; hand-lettered. The text in Fraktur and German script is in the center of the document. It is framed by a heart-shaped border. Above it is a winged angel head. On the sides are two stems of flowers. Along the top and bottom are borders. This bookplate is in the tune book for which it was made.  (PA GER Borneman 36)

This is one of four tune books made by Kulp in January 1812 featuring large winged angel heads.  Some of the tunes appear to have been entered a page at a time, with four tunes per page, as evidenced by changes in the penmanship.  There are 20 pages of unnumbered tunes and two pages each containing four rows of empty musical staffs.  The booklet has a hard cover with marbleized paper and  a leather spine.
