Gott Vatter voller Gnaden \ gib mir im tieffsten Grund zu erkennen den \ Schaden, drinn der Mensch liegt jetzund: Da er so gar verdorben, von inn und aussen sehr; an der \ Seelen gestorben; kein gutes hat allhier. Von der Sünd auferstehet wehtage und Kranckheit und \ all Kräffte durch gehet biß kömmet zum Abschied, Diß ist der Sold der Sünden davon niemand \ frey ist, unter den Menschen Kindern, all diese Zeit und frist. So der Mensch nicht gefallen, wä \ re solch böses nicht, darum um von dem allen, entstehet Kranckheit mit. Das zeigt der Seelen \ Schaden, die verdorben in grund. Das hat uns aufgeladen Adam biß dise Stund. Doch alles \ bey dir stehet, Kranck und auch gesund sein: Welches du hie verleyhest ist aus der Güte dein;

Translation:

God father, full of grace, grant that I recognize from the deepest depth the damage in which humans now lie, so completely spoiled very much from the inside and outside, dead in soul, has nothing good here. From sins arise days of woe and sickness and all strengths go away until comes the departure. This is the wage of sin from which nobody is free among the children of men all this time and term. If man had not fallen, such evil would not be. Therefore now among all of this arises sickness. That shows the soul’s damage which is spoiled to the bottom. This was brought onto us up to this hour by Adam. But everything stands with you, being sick and also being healthy, which you grant here is out of goodness.

Notes:

Hand-drawn; hand-lettered. The text in Fraktur, German script and Roman script extends across the document. The first two lines are particularly prominent. The first letter is written in an especially large and decorative style. Above the text and in the lower corners are calligraphic scrolled designs. On the motif in the lower left is a bird. The letters at the end of the text appear to be exercises in lettering.

The use of an English language verse following the German text on the fraktur is seen on many Vorschriften from Montgomery County.
