Translation:

This spiritually rich hymnal belongs to me, David Groff, was born on the 15th of November in the year 1804, in the sign of Aries. Oh Lord, teach me to ponder that I must die one day. teach me to prepare for the last closing of life. Present me death to me and awaken my desire that Jesus my prepare me in my still healthy days.
